OKMEYDANI SAĞLIK HİZMETLERİ LTD. ŞTİ.  As the Data Controller, we attach the utmost importance to the protection and security of your personal data. Accordingly, within the scope of the Personal Data Protection Law No. 6698 (“KVKK” or the Law), which was enacted to protect the fundamental rights and freedoms of individuals, especially the right to privacy, and to protect personal data, we would like to inform you, as the Data Controller, about the collection methods of your personal data, the purposes of processing, the legal grounds for processing, and your rights, within the scope of this Information Notice:

Purpose of Processing Personal Data: 

Your personal data processed within the scope of this Information Text is processed for the purposes of conducting communication activities and making your appointment within the scope of providing patient treatment services.

If there is any change in the purpose of processing your Personal Data specified in the Information Text, your consent will also be obtained.

Parties to whom Personal Data may be transferred and the purpose of transfer: 

Within the scope of the transfer of personal data in Articles 8 and 9 of the KVKK, your personal data is transferred to the online appointment software program.

Collection Method and Legal Reason 

Your personal data is collected electronically through our website to be used for the purposes specified in this Information Text.

In this context, based on the personal data processing conditions specified in Articles 5 and 6 of the KVKK, your personal data is processed within the framework of the data processing purposes specified in Article 1 above, for the legal reason that it is directly related to the establishment of a contract.

Rights of Personal Data Owners 

In accordance with Article 11 of the Law, as the data owner;    

• To learn whether your personal data is being processed,
• To request information regarding the processing of personal data,
  • To learn the purpose of processing personal data and whether they are used in accordance with their purpose,
  • To know the third parties to whom personal data is transferred, either domestically or abroad,
  • To request correction of personal data if it is processed incompletely or incorrectly and to request that the action taken in this context be notified to third parties to whom personal data has been transferred,
  • To request the deletion or destruction of personal data in case the reasons requiring processing are eliminated, even though the data has been processed in accordance with the provisions of the KVKK and other relevant laws, and to request that the action taken within this scope be notified to third parties to whom the personal data has been transferred,
  • To object to the emergence of a result against the person himself/herself by analyzing the processed data exclusively through automated systems,
  • You have the right to demand compensation for damages in case you suffer damages due to the unlawful processing of

your personal data.

Pursuant to Article 13/1 of the Personal Data Protection Law, to exercise your rights specified in this article, please submit your written request, complete with the application form and documents proving your identity, available at www.okmeydanitipmerkezi.com, either in person or via a notary public, to   “Gürsel Mahallesi, Darülaceze Caddesi, No:36 Ekşioğlu İş Merkezi Kat:6/9 Kağıthane, İstanbul.” Your application will be evaluated and finalized within 30 (thirty) days. Additionally, if you submit your request via email to  okmeydanisaglik@hs01.kep.tr,  your application will be evaluated and finalized within 30 (thirty) days.

If you provide a written response to your requests and an additional cost is incurred, you may be charged a fee according to the tariff determined by the Personal Data Protection Board.

Personal Data Protection Law (KVKK) Information

At Okmeydanı Private Medical Center, we value your security and, in this context, would like to inform you about the “Personal Data Protection Law,” which was enacted to protect individuals’ fundamental rights and freedoms, especially the right to privacy, and to protect personal data. As Okmeydanı Private Medical Center, in accordance with Law No. 6698 on the Protection of Personal Data (KVKK), we, as the “Data Controller,” may process your personal data within the framework described below.

1. Collection, Processing and Purposes of Processing of Personal Data

Private Okmeydani Medical Center,

Personal data processing is carried out in accordance with Article 20 of the Constitution and the principles of Article 4 of the Personal Data Protection Law. Private Okmeydanı Medical Center retains personal data for the period stipulated by law or required for the purpose of processing personal data. Data is collected to ensure that healthcare services are provided within the established legal framework and to ensure that Private Okmeydanı Medical Center fully and properly fulfills its contractual and legal obligations. Personal data may vary depending on the healthcare services provided, but it is collected verbally, in writing, or electronically through automated or non-automated methods, such as call centers, websites, online services, and similar means.

Your Personal Data;

Your data will be processed by our medical center, including, but not limited to, your name, surname, Turkish ID number, passport number or temporary Turkish ID number, place and date of birth, marital status, gender, patient number specific to our hospital, and other identification data that can identify you; your financial data such as your address, telephone number, e-mail address, and other contact data, payment and billing information; audio and digital information that may be obtained by us from time to time through electronic or non-electronic means; camera recordings if you visit our hospital; your vehicle license plate number if you use the parking lot; your general and specific personal data, especially personal health data obtained during the execution of all medical diagnosis, examination, treatment, and care services; your data related to private health insurance for the financing and planning of health services and your Social Security Institution data; your financial data such as your health and identification data, payment, and billing information that you submit through our website.

In addition, your personal data may be processed when you use our call center or website to use Private Okmeydanı Medical Center online services, when you participate in training, seminars or organizations organized by the Medical Center, or when you visit or navigate through web pages.

Your personal data will be processed in accordance with the personal data processing conditions and purposes specified in Articles 5 and 6 of the Personal Data Protection Law. Your special personal data, especially your health data, and your general personal data may be processed by our Medical Center for the purposes specified below.

Your personal data, including your special personal data, will be processed for the purposes of fulfilling our legal obligations stipulated in the Health Services Fundamental Law No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Regulation on Private Health Institutions Providing Outpatient Diagnosis and Treatment, the Private Hospitals Regulation, the Regulation on the Processing and Ensuring the Privacy of Personal Health Data, and other relevant regulations; protecting public health, providing preventive medicine, medical diagnosis, treatment, and care services.

Planning and management of health services and their financing;

• To inform you about the appointment if you make one;
• Planning and managing the internal operations of the Medical Center,
• Monitoring and preventing abuse and unauthorized transactions;
• Carrying out risk management and quality improvement activities;
• Conducting research;
• Billing for our services;
• Verify your identity;
• Confirming your relationship with institutions that have agreements with our Medical Center;
• Sharing requested information with private insurance companies within the scope of financing health services;
• To answer all your questions and complaints regarding our health services;
• To take all necessary technical and administrative measures within the scope of data security of our Medical Center systems and applications;
• Analyze your use of healthcare services and store your health data in order to develop and improve the healthcare services we provide to you;
• Preserving information regarding your health data that must be kept in accordance with the relevant legislation;
• Ensuring financial agreements with our contracted institutions regarding the healthcare services provided to you;

It may be processed for similar purposes, including, but not limited to, measuring patient satisfaction.

In addition, your personal data may be processed within the framework of Health Services Fundamental Law No. 3359, Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Regulation on Private Healthcare Institutions Providing Outpatient Diagnosis and Treatment, the Regulation on Private Hospitals, the Regulation on the Processing and Protection of Privacy of Personal Health Data, the regulations of the Ministry of Health, and other legislation. These data may be transferred to the physical archives and information systems of our suppliers and stored both digitally and physically. Furthermore, the data of our Private Okmeydanı Medical Center employees may be processed by us or by natural or legal persons we collaborate with or authorize, pursuant to the employer’s right to management, within the scope of the obligations stipulated by the Labor Law, Labor and Social Security Legislation, and other applicable legislation, for operational reasons such as increasing employee satisfaction and ensuring occupational safety and peace.

2. Persons and Organizations to Which Your Personal Data May Be Transferred

Your personal data will be processed within the scope of the Law and similar legislation and for the purposes stated above:

• Social Security Institution,
• Ministry of Health and its sub-units, Family Medicine Centers
• Private insurance companies (health, retirement and life insurance, etc.)
• General Directorate of Security and similar law enforcement forces,
• General Directorate of Population,
• Turkish Pharmacists Association,
• The courts,
• Laboratories, centers and similar third parties with whom we cooperate for medical diagnosis,
• Your authorized representatives,
• The healthcare institution to which the patient is referred or to which the patient applies,
• Third parties from whom we receive advice, including lawyers, tax advisors and auditors,
• Regulatory and supervisory institutions, official authorities

It may be transferred to our suppliers, support service providers, business partners and other third parties whose services we use or with whom we cooperate, within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of the KVKK.

3. Method and Legal Reason for Collecting Personal Data

Your personal data is collected, in all forms of verbal, written or electronic media, for the purposes set out above and to provide health services within the legal framework and to ensure that Özel Okmeydanı Medical Center fully and properly fulfills its contractual and legal obligations.

4. Your Rights Regarding the Protection of Personal Data

Regarding your processed personal data, in accordance with the legislation;
You have the right to learn whether personal data is being processed, to request information about it if personal data has been processed, to access and request personal health data, to learn the purpose of processing personal data and whether it is being used in accordance with its purpose, to know the third parties to whom personal data is transferred, whether domestically or internationally, to request correction of personal data if it is incomplete or inaccurately processed, to request deletion or destruction of personal data, to request notification of the third parties to whom personal data is transferred of the processes regarding correction of personal data if it is incomplete or inaccurately processed and/or deletion or destruction of personal data, to object to the emergence of a result to the detriment of the person by analysis of processed data exclusively through automated systems, and to request compensation in case of damages incurred due to unlawful processing of personal data.
